Membership Fee:
Junior Membership (under 18 years of age): $5.00
Senior Membership: $10.00
All membership funds will be deposited to the Association’s account and will be used solely in promoting the Association’s objects for which the Association is established and in exercising the Association’s powers.
Application for Membership:
Payment of the applicable membership fees must accompany this application.
Your application for membership to the Association will not be submitted before the Management Committee for approval until such time as you have paid the nominated membership fee. The Secretary of the Association will advise if your application for membership has been accepted or rejected by written notice as soon as possible following the next Management Committee meeting.
Resignation of Members:
You must provide written notice to the Secretary of the Association if you wish to resign as a member of the Association. Your resignation will take effect at the time that the notice is received by the Secretary, or at any time as stated in your notice.
Termination of Membership:
The Management Committee may immediately terminate your membership if you: -

are convicted of an indictable offence; or
-
you do not comply with any of the provisions of the Rules of the Association; or
-
you have membership fees in arrears for more than two months; or
-
you conduct yourself in a way considered to be injurious or prejudicial to the character or interests of the Association.
Before the Management Committee terminates your membership, the Management Committee will give you a full and fair opportunity to show why your membership should not be terminated. If, after considering all representations made by you the Management Committee decides to terminate your membership, the Secretary of the Association will give you a written notice of the decision.
Appealing Rejection or Termination of Membership:
You may appeal against the rejection or termination of your membership by giving the Secretary of the Association notice of your intention to appeal the Management Committee’s decision within one month of receipt of the Management Committee’s decision to reject or terminate your membership (“your Notice”).
The Secretary of the Association will call a General Meeting within one month after receiving your Notice. The General Meeting to decide your appeal will be held within three months after the Secretary receives your Notice.
You will be given full and fair opportunity to show why your application of membership should not be rejected or your membership should not be terminated. The Management Committee will have a full and fair opportunity to show why your application of membership should be rejected or your membership terminated.
Your appeal will be decided by a majority vote of the members present and eligible to vote at the meeting.
If you do not appeal against the decision within one month after receiving written notice of the decision of the Management Committee, or your appeal is unsuccessful, the Secretary will refund your membership fee to you.
Public Liability & Civil Liability Insurance:
The Management Committee advises that the Matrix Boxing Gym Inc currently holds public liability insurance up to $20,000,000.00 and civil liability insurance up to $5,000,000.00.
Discrimination
The Association takes discrimination and sexual harassment very seriously. Discrimination and/or sexual harassment by any member of the Association will not be tolerated and any member found to sexually harass or be discriminatory against another member will have their membership immediately terminated.
You must immediately advise a trainer or member of the Management Committee if you have been sexually harassed or discriminated against by any member of the Association. Your complaint will remain strictly confidential.
Your Responsibilities
You must use all equipment of the Association in the manner for which it was intended. You must immediately advise a trainer or member of the Management Committee of any damage to any equipment or to any area of the Association’s premises.
Should any injury occur, you must immediately advise a trainer or member of the Management Committee and seek assistance from them with regards to first aid or medical help. You must ensure that if any blood injury occurs you avoid contact with any other member of the Association to ensure infection does not occur.
In the event of an emergency, you must obey any trainer or member of the Management Committee’s direction.
I confirm that the answers given by me in the Health Inventory are true and correct in every particular.
I agree that any photos taken of me during training, gym socials or other gym related functions may be used in the Matrix Boxing Gym Newsletter or on the Matrix Boxing Gym website or in other material produced by the gym.
In consideration of the acceptance of enrolment to Matrix Boxing Gym, I, for my heirs, executors and administrators, to the extent permitted by law I release and forever discharge Matrix Boxing Gym and its’ directors, managers contractors and its’ servants’ employees and agents of all liabilities, claims, damages, costs or any expenses which I may have against them arising out of or in any of the parties.
I acknowledge that I have sole responsibility for my personal actions and possessions.
I have not participated in a fitness appraisal at Matrix Boxing Gym and do not hold Matrix Boxing Gym and its’ directors, managers contractors and its’ servants’ employees and agents responsible (to the extent permitted by law) for any personal injury, loss or damage which may occur as a result of my attendance at Matrix Boxing Gym.
